Application for Benefits
At the time a reemployed retiree becomes eligible for and chooses to
receive a benefit, the retiree should complete an Application for
Monthly Annuity or Lump-Sum Payment (Reemployed Retiree) or an Application for Lump-Sum Payment (Reemployed Retiree Under Age 65). If the retiree has been employed in the current year, an application
should be forwarded to each current-year employer for certification
of current-year earnings and contributions. The employer should
then forward the application to STRS Ohio.
