Retirement Incentive Plan for Defined Benefit Plan Members
Member Information Packet
The retirement Incentive Plan participant must complete a Service
Retirement Application and a Notice to Employer of Intent to Participate
in a Retirement Incentive Plan form and submit them directly to
STRS Ohio.
These forms are included in the Member Information Packet, which is available from STRS Ohio upon request. The complete Member Information Packet should be distributed by employers to eligible Retirement Incentive Plan participants.
The Member Information Packet includes:
- Retirement Incentive Plan brochure.
- Instructions — an outline of paperwork to be completed and other retirement-preparation information.
- Notice to Employer of Intent to Participate in Retirement Incentive Plan — to be submitted by member to employer; must be received by STRS Ohio before retirement date.
- Service Retirement Application — to be submitted by member directly to STRS Ohio before retirement date.
- Return Envelope.
