Request for Additional Information
Salary histories for the four- or five-year period preceding retirement have an impact on the computation of benefits for many retiring teachers. Therefore, employers are asked to complete a Request for Additional Information form if the member has an August-to-July contract or has had supplemental earnings in the five-year period preceding the member’s retirement.

The Request for Additional Information form can be completed online if you have registered to use STRS Ohio’s Employer Account Information area of the Web site. This form is sent to employers with the Deposit and Service Report. Both forms should be completed and returned together to STRS Ohio.

For members with an August-to-July contract year, STRS Ohio uses this information to reconcile contract year earnings to the amounts reported on the Annual Report of Member Contributions for each year. In some cases, a member’s final average salary (FAS) is based on contract year earnings versus the fiscal or annual reporting year. For members whose FAS is limited by Ohio law, this information is also used to determine the reason for the limitation and whether it meets allowable exceptions. See the FAS section in the Employers Manual for more information.

The Request for Additional Information form requests information for the five contract years preceding the current year.

For each year listed on the form, record in the appropriate spaces:

    Line 1: The position the member held for the respective fiscal years.

    Line 2: The amount of the member’s contract for the fiscal year.

    Line 3: Deducted amount for board-approved docked days.

    Line 4: Any amounts reported during the year but actually earned in the previous fiscal year.

    Line 5: The beginning dates of the respective contracts.

    Line 6: The ending dates of the respective contracts.

    Line 7: The number of days of service to be rendered under the member’s contract.

    Line 8: The percentage of pickup only if used in compensation for retirement purposes.

    Line 9: Amount of compensation for extended days. Extended days occur outside the beginning and ending contract dates.

    Line 10: Any supplemental earnings. List all supplementals separately, giving a brief description of each. Do not include summer earnings that are part of the member’s “regular” contract. These summer earnings would be recorded under contract earnings.

    Line 11: On this line, list any amounts earned during the fiscal year that were included in the Annual Report for work under a prior year contract. For example, the July earnings for a member employed on a 260-day, August-through-July contract are earned and should be reported on the Annual Report for the fiscal year following the contract year.

 

Contributions Prior to Retirement Date

Line 7: is used to record any taxed contributions that were not “picked up” during the fiscal year. This amount should be the same as the amount reported on the Annual Report of Member Contributions.

Line 8: must reflect any employee contributions “picked up” by the employer, whether through salary reduction or as a fringe benefit. This amount should be the same as the amount to be reported on the Annual Report of Member Contributions.

Line 9:  is the total employee contributions for the fiscal year. This is the sum of lines 7 and 8.

Note: After the effective retirement date, a member should be classified as a reemployed retiree, and any earnings after that date should be labeled as reemployed earnings on the Annual Report of Member Contributions.

 

Service Credit and Contract Information

Line 10: is used to report the amount of service credit earned by the member during the fiscal year and prior to the retirement date. Refer to Section 7 in this manual for instructions on calculating earned service credit.

Line 11:  should indicate the position held by the employee during the fiscal year.

Line 12: should be the most recent contract salary. The contract salary is the amount the individual would have earned if employed for the entire contract year, excluding supplemental earnings. If the member was granted a pay increase during the fiscal year, the reported contract amount should be the combined total of the two rates of pay for that year.

Line 13: should indicate the month and day service was contracted to begin and end.  Also indicate the number of service days in the full contract. The information shown should be for the entire term of the contract, whether or not the individual actually completed the contract.

Line 14:  should show the percentage increase generally granted to teaching employees in the current contract year over the preceding year. In accordance with Section 3301.501, Revised Code, the maximum allowable percentage increase in compensation for the two highest years (and any subsequent partial years) is limited to the greater of: (1) the member’s highest percentage increase in the preceding three years, or (2) the percentage increase generally applicable to all STRS Ohio members employed by the same employer.

The percentage shown on this line will be compared to the individual’s increase in the three preceding years to determine maximum allowable compensation used in calculating the final average salary.

The Deposit and Service Report for Employees With Continuing Employment is to be certified by the treasurer or another responsible fiscal officer. Please include a telephone number to help STRS Ohio complete any necessary follow-up.

Employers are reminded that the Revised Code strictly prohibits retirement contributions on any payments for unused sick leave or vacation or any other payments for services not actually rendered.
