Survivor Benefits

• Under both disability programs, if the member dies, eligible survivors may take monthly survivor benefits or a cash refund of the member’s account. However, under disability retirement, this cash refund is reduced by the amount of disability paid; it is not reduced under disability allowance.
• Under both disability programs, a one-time, lump-sum death benefit of $1,000 is payable to the qualified beneficiary (although the member may purchase an additional $1,000 or $2,000 death benefit). The beneficiary will be asked to submit a completed application and a copy of the death certificate.
