The Affordable Care Act (ACA) requires health care plans to report coverage information to the IRS for individuals who received minimum essential coverage during the year. The IRS uses the information to administer the individual shared responsibility provision of the ACA. As proof of compliance with this regulation, STRS Ohio or your plan administrator will provide you with a 1095-B form for 2016 tax purposes.

Will I receive a 1095-B form?
You will receive a 1095-B form if you were covered by an STRS Ohio health care plan at any time during 2016. You may receive this form from STRS Ohio or your plan administrator depending on which plans you were enrolled in during the calendar year.
Will STRS Ohio issue my 1095-B form?
It depends. STRS Ohio will issue 1095-B forms for enrollees who participated in a Medical Mutual plan for at least one month during 2016 who are:
- Under age 65 with no Medicare coverage;
- Age 65 or older with unconfirmed Medicare status; or
- Covered by Medicare Part B-only.
In addition, enrollees who participated in the Aetna Medicare Plan, but subsequently lost eligibility, may also receive a 1095-B form from STRS Ohio for the months their Medicare eligibility was being reviewed.
AultCare, HealthSpan and Paramount will issue 1095-B forms for their enrollees without Medicare. Medicare enrollees in these regional plans, along with Aetna Medicare Plan enrollees and Medical Mutual enrollees with Medicare Parts A & B, will receive 1095-B forms from the Centers for Medicare & Medicaid Services (CMS) or their plan administrators. When will I receive my 1095-B form?
STRS Ohio will mail 1095-B forms on Jan. 17, 2017. If STRS Ohio is issuing your form, you can access it in your STRS Ohio Online Personal Account on Jan. 13, 2017.
If STRS Ohio is not issuing your form, please contact your plan administrator for more information. Please note that the 1095-B form will not be available in your STRS Ohio Online Personal Account if it is issued from your plan administrator.
How many 1095-B forms will be issued for family coverage?
Benefit recipients will receive one form showing coverage for themselves and their covered dependents.
Will I receive multiple 1095-B forms if I was enrolled in more than one plan in 2016?
If you were enrolled in more than one plan in 2016, you may receive multiple 1095-B forms from different health care plans for specific months of coverage.
