Secure Document Uploader
You can securely upload electronic versions of STRS Ohio forms and other required documents through the Secure Document Uploader in your Online Personal Account. The files uploaded must be either a PDF or TIFF. Documents STRS Ohio will accept through this uploader include:

- Birth Certificate
- Death Certificate
- Direct Deposit
- Divorce Decree
- Guardianship Order
- Marriage Certificate
- Power of Attorney
- Proof of Medicare
- Service Retirement

If you want to email a form that cannot be submitted through the Secure Document Uploader in your Online Personal Account click here. Please note that email submissions on this site are unsecured, so only include your name and the last four digits of your Social Security number or STRS Ohio account number with your message.

STRS Ohio does not tolerate any type of fraud, waste, theft or abuse related to STRS Ohio benefits, health care coverage and business or other dealings with the retirement system. STRS Ohio will investigate all credible reports of suspected fraud, waste, theft or abuse and may pursue prosecution or other legal action if it finds evidence that fraud, waste, theft or abuse may have occurred.
Suspected fraud, waste, theft or abuse can be reported by calling the STRS Ohio Fraud Reporting line toll-free at 800‑677‑4038 or by sending the information in an email to [email protected].
STRS Ohio will not retaliate against any person/entity for making a report in good faith. Baseless allegations, made with disregard for truth or accuracy, or frivolous complaints will not be tolerated. People making such allegations may be subject to legal actions by the individuals accused of fraudulent conduct.
