Deposit and Service Report

Overview

The deposit and service report is an interim report that employers complete for all members applying for service retirement or disability benefits, or for eligible survivors. The information on the deposit and service report is used by STRS Ohio to calculate benefit payments.

Employers will receive an email notification when deposit and service reports are available to complete. These reports are completed online in ESS. STRS Ohio sends deposit and service reports to employers as follows:

  • Service retirement applicants — Approximately 60 days before the scheduled retirement date.
  • Disability benefit applicants — After the STRS Ohio examining physician recommends that a disability benefit be granted.
  • Survivor benefits — As soon as possible after STRS Ohio is notified of the member’s date of death.

A deposit and service report should be submitted to STRS Ohio as soon as the member’s earnings and contributions can accurately be calculated but no later than two weeks following the member’s retirement effective date.

STRS Ohio has two checklist worksheets available to help employers consolidate all required data before filling out the deposit and service report and request for additional information. Use the Retirement Reporting Checklist (Full-Time or Part-Time Employee) or the Retirement Reporting Checklist (260-Day Employee) to gather all needed information. You can also use the paper deposit and service report form to collect information, but all deposit and service reports must be completed in ESS and sent electronically to STRS Ohio.

Request for Additional Information

Salary histories for the five-year period preceding retirement have an impact on benefit calculations for many retiring educators. Employers are asked to complete a request for additional information form in ESS if:

  1. The member had supplemental earnings in addition to contract earnings; or
  2. The member’s contract year is different than the STRS Ohio fiscal year of July 1 through June 30.
